
REGISTRATION FEE:
Early
Bird (by December 1): $600. After December 1: $675.
Registration Fee includes two nights' lodging (regular
room) for course participant and one guest, all course materials, reception, Saturday
breakfast and refreshments indicated on Program.
REGISTRATION DEADLINE:
December 28, 2008. Event dates: Janaury 23-24, 2009.
CANCELLATION:
Refunds except for $50 administrative fee will be made before
deadline. After deadline, fee is non-refundable. We reserve the right to cancel
the meeting; full fee will be refunded.
CONFIRMATION:
Confirmed registrants will receive notification within two weeks
of registering. If you do not receive a confirmation notice, please contact

CONTINUING EDUCATION:
Our joint sponsor, The
Catholic Medical Center is accredited by the New Hampshire Medical
Society Council on Education to provide continuing medical education
for physicians.
The
Catholic Medical Center designates this continuing medical educational
activity for a maximum of 8 credit hours in Category 1 credit
hours toward the AMA Physician’s Recognition Award. Each
physician should claim only those credits that he/she actually spent
in the educational activity.
